How Do I Request a Partial Transfer of My STRATA IRA to a Successor Custodian?

Transfers are initiated by the receiving custodian. Please complete the successor custodian’s transfer request form. The completed form must include clear asset delivery instructions, the account holder’s signature and a letter of acceptance signed by an authorized signer of the successor custodian firm.
